Chronic Disease Management (CDM) Plans
Eligible patients may receive a Medicare rebate of $61.80 for Osteopathy appointments under a GP Chronic Disease Management (CDM) Plan. GPs at Dockside Medical Hub can provide the required referral documentation, with consultations bulk billed where applicable.
CDM services are subject to Medicare eligibility criteria and GP approval. Please check with your GP to confirm eligibility and referral requirements.
